1.How do I make an appointment?
You can schedule an appointment online by clicking the "Request an Appointment" button, the appointments tab or you can call us at 225-255-0899. Please leave a voice message if we are unable to take your call.
2. What documents will I need to complete the registration forms?
You will need your insurance card, unexpired government issued photo ID, medications, pharmacy and medical provider's contact information. If you would like to upload health records the instructions are located within the patient portal. If you are having difficulty completing the forms, click the link near the top of the page to obtain detailed instructions.
3. What should I expect during my first visit?
Your appointment will be conducted online using a secure confidential audio-video platform. Therefore, you will need access to a computer, smart phone, or tablet to attend the appointment. Please login at least 5 to 10 minutes prior to your scheduled appointment time. We recommend testing your audio and video after logging into the session. We also request that you are not driving during the appointment and are out of bed and dressed.
4. How can I pay my bill?
All payments are due at the time of service. A link will be provided so that the patient is able to pay their bill and copay online.
5. How do I request a medication refill?
It is preferred that you send a message through the patient portal to request a medication refill. Refill requests should be sent at least 5 days prior to taking your last pill.
